I'm using them on flyers at the moment, but I'm hesitant to start using them on yard signs. I think they're a bit ugly, so when I do insert them I try to make it as on obtrusive as possible. I love the concept, but I don't think it's going to be the long term winner for bridging print and online connections. Someone is bound to make a far more elegant solution (augmented reality is already showing promise in that regard)
QR codes are the current "it" thing right now, but they will certainly be replaced within a few years. I like them on my yard signs, but I've been finding that they are indeed limited use. I'm working on creative ways to use them in marketing to get people to squeeze pages, so we shall see how well that works!
I doubt you could ever see that on an established blog. In order to get 50% traffic from Facebook, you would need a massive campaign of some sort to get there, and I think any established blogger sees most of their traffic from Google.
