Nice and simple.
If I may ad, when preparing an action plan to present to your boss, make sure you know what HE expects to find in it. We must try to include in some small way a few objectives or values that our superiors want to see. I worked hard on great actions plan that I had to restart because it didn't mean a thing to my boss. It is our job to know what the companies objectives and plans are and design your plan around it.
