Based on the single neck wound she had, I'm guessing she was attacked in the forest but managed to get away. Perhaps she ended up in that abandoned farmhouse to escape and died there, leaving Otis to find her on one of his hunting trips. It was a killer ending. I'm still wondering what the heck the CDC guy whispered to Rick in season 1. Many viewers thought he was telling him about Lori's pregnancy but we know now that's not it.
