I personally dislike the whole coma theory. I think her living seems like the typical happy ending cop-out you'd expect, but if you look closer it's very much a punishment in the "be careful what you wish for" variety. She always wanted peace and had fought tooth and nail for it, but then acknowledged in the final Arc that she thrived in conflict and functioned best with a distinct goal in mind. The only fulfilling conclusion for someone like that is death. The worst thing that could happen to them is to get exactly what they wished for: peace. She is completely powerless (in more ways than one), she has no more conflict and no more goals. She's forced to suffer through the process of learning how to be a normal human being. At the same time though, she finally has a chance to have a normal relationship with her father and to experience the reprieve she'd always wished for but never truly wanted. Much like the rest of Worm, it's bittersweet.
Every Arc had at least one planned Interlude chapter, but there are a lot of Interludes that were "activated" through reaching a donation threshold. I'm a bit unsure about the details of the Epilogues myself, but I'm fairly certain they were all planned and not donation-based.
