In the same vein, a leader who comes out of a meeting saying, "Right: we'll do this!" and then turns around and does something different (usually via email or text) is also a leader others won't follow.

How about these:
- shut your eyes to the talents of others in the congregation
- equate "growing the church" with "growing the Kingdom"

You struck a nerve here, Ron, largely because I did that very thing - blast a pastor - the one I'd been serving under for years - via email. We'd had a falling-out, and while I spent a lot of time trying to come up with the right words -- and even consulted with another pastor on how to say it -- the bottom line was, I didn't have the cojones to speak to him face-to-face. Praise God, we did make up, face-to-face, and move forward (which was good, because less than three months later, he was dead from cancer which had gone undiagnosed), but I still shake my head at my own cowardice. Someone once suggested asking yourself four questions before hitting "send": 1 - is it true? 2 - is it kind? 3 - is it clear? 4 - is it necessary?
