Not to sound like a dude who doesn't understand the interwebs... but do you mean a website? Lol. I just started creating a website on hypnosis! I'll let you know as soon as it's ready.

You mentioned "getting lost" in the middle. That's a good sign!
I intentionally did what's known as a "confusion pattern" near the middle to get your conscious mind out of the way for a few minutes, and then deliver some suggestions meant more for your unconscious mind. It's not really meant for anyone to follow consciously... In fact, the point is to leave the conscious mind confused and trying to follow the train of logic... But your unconscious mind takes it all in and utilizes it.
In my experience, it tends to help people get the suggestions at a deeper level.
As for your problems...
I have a quick technique for you when you're about to make a call. It sounds silly at first, but it's a great technique for any anxiety. I've used it to great success with a few telemarketers that were afraid of the phone!
You may need to walk through this process a few times, but once your mind gets it, you're going to feel great!
Ok... the next time you have a call coming up, and the next time you think you're going to feel a lack of confidence beforehand... Take a minute and imagine the call 100% successful. Close your eyes, imagine travelling into the future and having the call and nailing every single point. Anything less than complete success (better than you can imagine) isn't worth imagining... So really go for the gold here. Imagine it as if you're inside your body, already feeling confident when you start, and make it completely successful when you imagine it. Then, when you're all the way through, imagine a ripple starting from the end of that event, back towards now, re-evaluating and re-arranging all the events to line up with your success. And the same ripple also going forward in time, making things after the call line up perfectly as well.
I know it sounds a little silly at first, just imagining it completely successful... But think about what you have to do in order to create the lack of confidence in the first place. Correct me if I'm wrong, but I'm betting you're imagining yourself sitting/standing there, taking the call and failing miserably, maybe tripping over every word and messing it up, or maybe saying all the wrong things to the person on the other end (unconsciously, of course). And that's what's causing the anxiety before starting the call.
But as you're well aware, you do great once you get started. So instead of imagining all the crap you could do wrong, imagine it going unexpectedly well... And you might find your confidence grow in that area with only a little effort!

I believe I set it years ago when I was commenting on a programming forum. I keep meaning to change it! Thanks for the reminder :)

Glad you enjoyed it! And wow, that is a huge question! I'll do my best to explain my thoughts on something that could probably be an entire book. I’ll also try to bypass the many belief systems on this subject as much as possible, and try to explain my thoughts from a practical rather than dogmatic point of view… But keep in mind these are just my humble opinions. Take from it what you feel will work for you. :)
Everyone seems to agree that our genetic past comes from at least 2 places, more depending on your beliefs. At the very least, there are physical genes we inherit (like hair color or freckles), and there are emotional/behavioral genes we inherit from our parents. The behavioral and emotional inheritance is something you could also say is simply learned rather than inherited. This is the type of inheritance I’ll focus on.
I’m a fairly scientific person. I like my beliefs to be based in facts… so when someone asks me about past lives and their effects, my true belief is simple… “I don’t really know.” I’ve done past life regression myself, experienced several really interesting lifetimes… But I still can’t say for sure. I’ve also had clients in hypnosis tell me that they felt like their current behaviors in life stemmed from a decision they made 15 lifetimes ago – and every lifetime since was a failed attempt to redeem themselves for one event 15 lifetimes ago! I’ve also had a client steadfastly refuse to believe in past lives… all while they actively resolved something their unconscious mind told them came from their great grandmother (it worked great for him).
In effect, beliefs and emotions can certainly be passed down the line (by simple learning at the very least). However, the behavior exists now, not in the past. Using hypnosis and NLP, it’s possible to release the decision or emotion behind the unhelpful behavior, and install a new one that aligns with what we want in our life.
In other words, you can learn all kinds of behaviors from all sorts of places, but the only person who can really determine your behavior is you, in the present moment!

You’ve got it. :) Sometimes that base belief of “I can do _____”, or even, “I can learn how to _____” is all you need to get started. Every once in a while I talk to a client who says, “I can’t do _____” and when I ask what’s stopping him - he replies that he’s been too scared to learn, or even try! That’s where we enhance his confidence... then discuss what he's going to do about it! Feelings lead to your actions, which lead to your results.
Thanks for your insight. Let me know how the video works out for you!

Thanks! Glad to hear you liked it! I totally agree. It's amazing the results you can get when you take a moment to thank your unconscious for the pain. After all, pain is a message that there's something to change. And so many people take ibuprofen when they get headaches rather than dealing with the root cause. Not that ibuprofen is all bad... but if that's the only thing you EVER do, that is the equivalent of saying, "Shut up, unconscious mind! Don't tell me what to do!"
Keep up the hypnosis!
