They weren't just "diagnosing" and playing "pretend-doctor". They were providing doctors' notes which are a work product of... doctors. If a "pretend doctor" provides a "pretend patient" with a prescription for oxycontin, I can guarantee you he wouldn't be appearing before a "pretend judge".
That is not the macho response, it's the childish response. Sheen is not a man, he's a boy stuck in a man's body, lacking the maturity that's needed to face the challenges and temptations that money, fame, and--on a more basic level--adulthood present.
Been a while since I've seen the show, but... these guys aren't real men. True, they aren't pantywaist metrosexuals, but they're not real men either. Being a real man entails two primary characteristics: masculinity and maturity. So, whereas metrosexuals are lacking in the masculinity department, these guys are lacking in the maturity department. They rage like two-year-olds. They cuss like high schoolers. They're self-centered. They lack self-control. They're little boys trapped in grown-ups' bodies. Unfortunately, these are the types of "men" that our culture promotes as some kind of ideal. No thanks.
