Unfortunately this post is a bit dated. Since WordPress 3.3, the performance issue with having /%postname%/ permalinks has been fixed. I now recommend that format for many cases, though sites that publish lots of content or want to get in to Google News should take a different approach.
I highly recommend Joost de Valk's post and .htaccess redirects generator for those who want to switch to the /%postname%/ format: http://yoast.com/change-wordpress-permalink-struc...

I've thought about using Readability and also tried Read It Later for awhile. The problem is I'm a bit of a digital hoarder so I collect things I find online that I'll never end up reading...but I love collecting and could spend all day doing it!

At least, this is what would match the website you linked to.
I don't think it's easy because I don't think most people want to read let alone write with gray on black text :)

As you note in your post, it's a pretty common method of hardening WordPress to install in an unusual directory, hide the meta generator tag, etc.
Conversely, I find it more interesting the other candidates haven't taken every precaution they could, given how I'm sure certain groups would love to take down or hack Republican candidate websites...

To answer your first question, Brett Terpstra has posted an excellent tutorial and sample about this: http://brettterpstra.com/adding-a-tinymce-button/
I should also write a post soon about a method I've used to add buttons to the HTML editor, for those who don't use the visual editor.
I think you could also use the method Brett writes about to add an upload/insert button to the visual editor area.
