The purpose of a prison sentence for a crime this vicious should be two-fold: (1) taking a dangerous person out of circulation to protect society and (2) punishment - this fails miserably on both counts. Does anyone believe he'll be less of a threat in 14 years?

Office complacency run amok? Maybe it just gets to be part of the culture and when enough people are doing it, no one thinks anything about it? I have no idea. 'What were they thinking?' is a good question, though. 'Not much', would be a good answer.
